Advice
We start by opening a portret, it helps when there is no background with the image, but when your at a stage you've got the hang of it you could even do it with images that have a busy background.
I found this one of a girl which I took from sxc.hu.

Step 1: Duplicating
You start of with duplicating the image.

Step 2: Lasso
Then with the second image selected you take the lasso tool and make your selection, you dont need to be very precise because you will be adjusting the second image further on in the tut.

Step 3: Warping
When you have selected all around the mouth press crtl + T, then right mouse click and select warp. Now you can start moving the lines around untill you are happy with them. Try not to go over the chin line with the lips.
When thats done double click or click on the move tool and then click apply.

Step 4: Erasing
Now you need to get the eraser tool, I took a softer eraser so the face blends in with each other. You can erase bits that you are not happy with cause you want to make it look "natural". So blending is the word. This is why you needed to duplicate the first layer so layer 1 will fill up the erasing from layer 2.

Step 5: Repeating steps
Now all you need to do is do those steps with each eye and your done.
